Application forms

Before starting your new service you may have to fill out one or more of the forms below. We've provided more information about each form, so you know which is relevant to your service and business.

• Register as a service provider
• Register as an 087 service provider 
  If you are an 087 service provider, you will need to register with PhonepayPlus by 31 July 2009. You are an 087 service provider if you have a contract with a network operator for the provision of an 0871, 0872, or 0873 number range; this EXCLUDES the 0870 number range.
• Register services/ Amend registration information
• Register as an information provider
